Where to Get Tax Forms
Tax forms not available on our Web site may be obtained from the following sources:
- Customer Service Center - Call 609-292-6400. Representatives are available from 8:30 a.m. to 4:30 p.m., Monday through Friday, except holidays.
- Automated Tax Information System (ATIS) - 1-800-323-4400 (toll-free from Touch-tone phones within New Jersey, New York, Pennsylvania, Delaware, and Maryland) or 609-826-4400 (Touch-tone phones anywhere). Order tax forms through the Forms Request Service 24 hours a day, 7 days a week.
- NJ TaxFax - 609-826-4500. Call NJ TaxFax from your fax machine's phone, follow the instructions to order the form you want and have it faxed to you instantly. NJ TaxFax is available 24 hours a day, 7 days a week.
- Regional Offices - Many tax forms can be obtained at our 7 walk-in offices located throughout the State. Click for the address of the Regional Office nearest you. Normal hours of operation are 8:30 a.m. to 4:30 p.m., Monday through Friday, except holidays.
