Here's How to Play
This ticket features three (3) separate game plays. In GAMES 1 and 2, the player gently removes the scratch-off material covering the play areas to reveal two (2) rows of game play. In each game, if YOUR NUMBER matches THEIR NUMBER, the player wins the PRIZE for that GAME. The player can win in both GAMES 1 and 2. In GAME 3, the player gently removes the scratch-off material covering the play area to reveal six (6) prize amounts. If three (3) of the six (6) prize amounts are identical, the player wins that amount. In GAME 4, the player gently removes the scratch-off material covering the play area to reveal two (2) WINNING NUMBERS, ten (10) YOUR NUMBERS and ten (10) prize amounts. If any YOUR NUMBERS match either WINNING NUMBER, the player wins the PRIZE shown. If the word LIFE is revealed, the player wins $2,000 A WEEK FOR LIFE.

How to Claim $1,000 Prizes and "$2,000 A Week For Life" Prizes!
When you win a $1,000 prize or a "$2,000 A WEEK FOR LIFE" prize, first sign your name on the back of the ticket, then fill out a Claim Form Mailer at any New Jersey Lottery Retailer location and mail the original Claim Form Mailer, along with the ticket, to Lottery Headquarters. For $1,000 prize winners, a check will be mailed to the winner following validation of the ticket. Winners of the "$2,000 A WEEK FOR LIFE" prize must provide a valid birth certificate for purposes of determining the winnerÕs age. Following validation of the ticket, the "$2,000 A WEEK FOR LIFE" prize (guaranteed minimum prize: $2,000,000) will be issued quarterly in advance starting on or about the anniversary date of the claim and will continue for the life of the winner. Neither the "$2,000 A WEEK FOR LIFE" prize nor any benefits attached thereto will be paid to any person under the age of eighteen (18) years. Rather, payment of the "$2,000 A WEEK FOR LIFE" prize will be deferred until the winner has attained eighteen (18) years of age.
