Outcomes Toolkit is an outgrowth of the Accelerating Community Transformation Project (ACT), a major effort to evaluate community-wide efforts to improve the quality of life and health in a community, and determine how these efforts can be accelerated. The focus is to work with a cross-section of community leaders to rethink the ways quality of life and community services are conceived and delivered. Outcomes Toolkit includes a flexible array of indicators, strategies and tools to enable coalitions to monitor and adjust their efforts to improve community quality of life and health. It provides users with an ability to communicate strategies and progress to a wide range of community members. Communities using Outcomes Toolkit select specific indicators, including many public health indicators, that represent their key goals and reflect the overall community vision. Each community sets target objectives for the future and tracks progress toward these goals. Outcomes Toolkit was developed by Macro International and Health Forum collaboratively with ACT communities and with leaders from approximately 20 additional communities from around the country. Originally designed as a distributed database systems, Outcomes Toolkit is currently being converted to a Web-based application.
