Formula Grants - Standard Form 269 (SF-269)
AoA’s formula grants have a one year project period however
an additional two years to expend these funds is allowed because
of the hierarchical structure of the aging network’s funds
distribution system.
A Financial Status Report (SF-269) is due semi-annually 30 days of the end of the 2nd and 4th quarters (April 30 and October 31) through the end of the three years the grant remains active.
The SF-269 can be completed below:
- OMB Standard Form (SF) 269
Final SF- 269
A final SF-269 is due 30 days after the project end date.
This report forms the basis of the close out of the grant. Any
project funds that remain unused at the end of the budget period
as reported on the final SF-269 are returned to the Treasury,
unless the grantee has requested an extension of the time available
to expend the funds during the time the grant was active.
The discussion of various types of requests for extension can
be found below:
- OMB Standard Form (SF) 269
