Native American Title VI Grants - Standard Form 269 (SF-269)

Title VI Grants require that six-month and twelve-month Financial Status Reports (SF-269) be submitted for each year within the three-year project period. The report can be completed here.

Requests for continuation and requests to carry over Title VI grant funds from one budget period to the next for each of the two initial years of the three year project period must be submitted on an annual basis in order to continue participating in the grant program. The form required to file a request for continuation and to carry over funds can be found below:

Final SF-269

The final SF – 269 Financial Status Report for Title VI grants is due 90 days after the 17th month, which is the project end date for the three year project period. A schedule of reporting requirements for the most recent Title VI grants can be found

The final SF-269 report when submitted to AoA forms the basis of the close out of the grant. Any project funds that remain unused at the end of the budget period as reported on the final SF-269 are returned to the Treasury. No funds can be carried over past the end of the project period.

If the unexpended funds as shown of the SF-269 have already been drawn down from the Payment Management System of HHS’ PSC, they must be returned to AoA. Use of unexpended funds after the grant has expired without an approved extension, will result in an audit disallowance.
