A replacement program letter and (order) postcard (Form 12815) to request 2008 materials will be mailed the week of November 24, 2008. You must use the new pre-printed postcard to place your 2008 order. The earlier postcards cannot be used to place an order.  We apologize for the inconvenience.

The (order) postcards issued early November contains a postcard that did not display the pre-printed account related information needed to process your order.
