Mobilizing Communities: A Compendium of AAPI Model Practices

This booklet discusses reducing health disparities and improving access to health care for Asian American Pacific Islander (AAPI) communities. It describes programs that address health care and areas of improvement, and highlights 13 AAPI health service models for the AAPI population. Topics include target population, project goal, project description, AAPI's served, languages, annual budget and funding sources, partnerships, health-related outcomes, and lessons learned.
