Abstract:

This working paper describes three project sites in Sheridan, Wyoming; Cambridge, Maryland; and Shady Grove, Oregon that were successfully able to continue providing services well after the expiration of their Rural Health Outreach Grant Program funds. It examines the reasons why each of these particular sites was successful in sustaining post-grant services. The paper dedicates a section to each project and includes tables and references.
