Hispanic Heritage Month 2008 Model Programs

Hispanic Americans are the largest minority population in our country, and this population is expected to continue to grow significantly.  As AoA honors and celebrates National Hispanic Heritage Month, September 15 through October 15, 2008, we are pleased to highlight programs and initiatives in communities that are providing culturally relevant and appropriate information and services to Hispanic elders and their families.  The projects selected highlight how the Tomando Control de su Salud, the evidence-based Stanford University Chronic Disease Self-Management Program for Hispanic elders is being implemented in communities across the country.

These model programs are only a representative sample of the outstanding work being done around the country to assist and engage Hispanic elderly.

Hispanic Americans have strengthened our country and contributed to the spirit of America. National Hispanic Heritage Month is an opportunity to honor these contributions and celebrate the rich cultural traditions of our Hispanic-American community.
