    Cruise Ship's Corrective Action Statement

Item No.: 02
Site: Medical-
Violation: On the cruise from 11/6 to 11/9/08, there were 2 ill crew members. The crew positions were not indicated on the gastrointestinal illness log.
Recommendation: The gastrointestinal illness surveillance log entry for each crew member shall contain crew member position or job on the vessel.

Violation: There was a crew member who reported to medical on 10/31/08. The gastrointestinal illness log indicated 3 episodes of diarrhea, but the crew member was listed as a non-reportable case. There were 2 passengers who reported to medical on 10/27/08. The gastrointestinal illness log indicated 3 episodes of diarrhea for one passenger and 5 episodes for the other passenger. Both were listed as non-reportable cases.
Recommendation: A reportable case of gastrointestinal illness shall be defined as: (1) Diarrhea (three or more episodes of loose stools in a 24 hour period); or (2) Vomiting and one additional symptom including one or more episodes of loose stools in a 24-hour period, or abdominal cramps, or headache, or muscle aches, or fever (temperature >38 °C (100.4 °F)); and (3) Reported to the master of the vessel, the medical staff, or other designated staff by a passenger or a crew member.

Site: Potable Water-Whirlpool #3 Machinery Room
Violation: There was a constant drip at the intermediate vent of the backflow prevention device. The vessel had identified the situation and the whirlpool had been emptied.
Recommendation: Ensure that backflow preventers are maintained in good repair.

Violation: There were 2 crew members ill on the 11/6 - 11/9/08 cruise. The isolation records indicated "isolated until end of cruise" and did not give a time of the end of isolation.
Recommendation: Ensure that symptomatic food employees meeting the case definition for AGE are isolated in cabin or designated, restricted area until symptom-free for a minimum of 48 hours.

Violation: Combination oven 495 had been out of service since March 27, 2008. According to staff, the wrong part was delivered and they are awaiting the correct one.
Recommendation: Ensure that food contact equipment is maintained in a state of repair and condition that meets the materials, design, construction, and operation specifications of these guidelines.

Violation: There was a gap underneath the grease collection spout on the flat grill.
Recommendation: Ensure that nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Violation: There were cables draping the bottom of the technical compartment below the coffee machine. This was corrected.
Recommendation: Ensure that nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Violation: There was a small amount of debris on the bottom front of the ice maker. The debris was on the exterior of the equipment and did not impact the ice bath.
Recommendation: Ensure that food-contact surfaces of equipment and utensils are clean to sight and touch.

Violation: The paper towel dispenser for the handwash station was located on the opposite side of a shelf from the handwash sink. Crew members would have to climb under the shelf in order to access the paper towels. According to staff, the shelf was newly installed.
Recommendation: Ensure that a handwashing facility includes a sink, soap dispenser, single-use towels dispenser, and waste receptacle.

Violation: The decks in the galleys and bar pantries were worn and difficult to clean due to chipped and peeling surfaces, especially at equipment foundations, gutterways, and deck/bulkhead junctures.
Recommendation: Ensure that decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are constructed and maintained for easy cleaning.

Violation: The deck/cabinet juncture was not coved. There was also soft sealant at the deck/bulkhead juncture at the far end of bar.
Recommendation: Ensure that bulkhead and deck junctures are coved (including galleys, pantries, deck/counter junctures at buffets, bars, waiter stations, and dining room work counters).

Recommendation: Ensure that the presence of insects, rodents, and other pests is effectively controlled to minimize their presence in the food storage, preparation, service areas, warewashing, and utensil storage areas aboard a vessel.

Site: Children Area-Diaper Changing Stations
Violation: The wash hands often sign stated to wash hands "before and after diapering" and not after each diaper change.
Recommendation: Ensure that signs are posted in the diaper-changing area advising child-activity center staff to wash their hands after each diaper they change.
