Item No.: 08
Site: Potable Water-Serenity Equipment Room
Violation: A double check valve assembly was installed on the potable water fill line for the whirlpool.
Recommendation: Replace the double check valve assembly with a backflow prevention device that has an intermediate vent.

Site: Potable Water-Bunker Chlorine Station
Violation: The potable water line next to the chlorine make-up tank was not striped blue. This line did not have a backflow preventer installed. A hose was connected to this line.
Recommendation: Ensure that backflow preventers are installed when air gaps are impractical or when water under pressure is required.

Site: Potable Water-Bunker Chlorine Station
Violation: The potable water line next to the chlorine make-up tank was not striped blue. This line did not have a backflow preventer installed. A hose was connected to this line.
Recommendation: Ensure that potable water piping and fittings are painted or striped auxiliary blue, or in accordance with ISO 14726, at 5 m (15 feet) intervals on each side of partitions, decks, and bulkheads in all spaces, except where the decor would be marred by such markings.

Site: Whirlpool/Spa-Water Works
Violation: There were a few days in the past month that the shock halogenation for the water works was not recorded. Prior to September 12, 2008, super chlorination was not recorded for the water works. Also, shock halogenation was not recorded for the whirlpools when they closed early.
Recommendation: Ensure that a written or electronic record of whirlpool spa filter inspection, backwashing, core sample sedimentation test, and shock halogenation (concentration and contact time) is maintained and available for review during inspections.

Site: Medical-
Violation: The time recorded for the initial interview with the close contacts of crew members ill with gastrointestinal illness was the time the contact was identified by the ill crew member and the contact's name was entered into STARDOCS. The nurse stated that a paper notice was given to the contact by the ill crew member and the supervisor of the contact was notified, but the contact is not initially spoken to by medical personnel. For the 24 and 48 hour interviews, medical personnel stated that they speak to the contact on the phone.
Recommendation: Ensure that medical staff conduct verbal interviews with asymptomatic cabin mates or immediate contacts to confirm their condition, advise them of the hygiene and handwashing facts, and instruct them to report immediately to medical if they develop illness symptoms.

Site: Preparation Room-Potato Room
Violation: The first potato peeler had a gap around the chute where the gasket was recessed. The second potato peeler had holes around the inside of the chute.
Recommendation: Replace the gasket and seal the holes.

Violation: The heating coil in the condensation pan of the cooling unit closest to the door was not working. There was ice in the condensation pan.
Recommendation: Ensure the heating coil is operating to prevent ice build-up.

Violation: The left center section of the curtain was damaged in walk-in refrigeration unit A 26 F.
Recommendation: Replace the damaged curtain.

Site: Galley-Hot Lines
Violation: The drip tray housings for the flat grills were deep and narrow, making cleaning very difficult.
Recommendation: Ensure that nonfood-contact surfaces are free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.

Site: Buffet-Poolside Grill
Violation: There were two unsealed pipe penetrations in the outside of the counter to the left of the handwash sink.
Recommendation: Install flanges on the pipe penetrations.

Violation: There were unsealed openings behind the coved tile and to the left of the ice cream freezer that led to the void space inside the bar counter.
Recommendation: Seal the openings in the bar counter.

Violation: The pressure gauge on the dishwash machine was not working.
Recommendation: Ensure that warewashing equipment is maintained in good repair and proper adjustment including: (1) Being maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; and (2) Maintaining water pressure, and water temperature measuring devices in good repair and ensuring their accuracy within the intended range of use.

Site: Galley-Dishwash
Violation: The left sprayer on the final rinse spray arm was clogged. This was corrected.
Recommendation: Ensure that warewashing equipment is maintained in good repair and proper adjustment including: (1) Being maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; and (2) Maintaining water pressure, and water temperature measuring devices in good repair and ensuring their accuracy within the intended range of use.

Violation: The thermometer on the potwash sanitizing sink indicated 170°F and the water was measured at 187°F.
Recommendation: Ensure that water temperature measuring devices: (1) That are scaled in Celsius or dually scaled in Celsius and Fahrenheit are designed to be accurate to ±1.5°C in the intended range of use. (2) That are scaled only in Fahrenheit are designed to be accurate to ±3°F in the intended range of use.

Violation: The technical compartment for the undercounter blast chiller was soiled with a black slimy material on the pipes and the floor of the compartment.
Recommendation: Ensure that nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Violation: The profile strip at the left side of the double salamander was not attached well and there were seams between it and the salamander.

Violation: A refrigeration unit in each pantry was not in use. Per staff, they had never been used and would not be used.

Site: Integrated Pest Management-
Violation: The integrated pest management plan was revised April 29, 2006. There was no indication that it had been reviewed for effectiveness in the past two years.
Recommendation: Ensure that the vessel's Integrated Pest Management Plan is evaluated for effectiveness every 2 years or whenever there is a significant change in the vessel's structure such as a renovation or operation.

Site: Housekeeping-
Violation: In the ladies toilet room near the casino, there was a handwash sink inside the handicap stall, but there were no door tissues to exit the stall. Only air dryers were available to dry hands.
Recommendation: Equip public toilet facilities so that persons exiting the toilet room are not required to handle the door with bare hands. This may be accomplished by methods such as locating paper towel dispensers at sinks and waste containers near the room door, installing mechanically operated doors, door removal, or other effective means.
