    Cruise Ship's Corrective Action Statement

Item No.: 02
Site: Medical-
Violation: The clinical sample collection containers for bacterial agents were expired.
Recommendation: Ensure that at least 10 clinical specimen collection containers for both viral and bacteriological agents (10 each), as well as the proper shipping containers and labels for same, are maintained by the medical staff.

Site: Potable Water-Production
Violation: The vessel has a variance to bunker water in Bermuda. A copy of the variance was on the vessel, but the engineering department did not have a copy. The vessel has been chlorinating the production water around 2.5 ppm. The variance agreement indicates that water produced while at dock in Bermuda should be chlorinated to at least 3.0 ppm. Additionally, the reverse osmosis units had been disinfected and flushed after each departure from Bermuda. Microbiological sampling was conducted in accordance with the variance and there were no positive sample results after production.
Recommendation: Ensure that all actions required by the variance are performed and recorded and maintained for at least 12 months.

Site: Potable Water-Crew Whirlpool Machinery Room
Violation: There was no air gap at the fill line to the whirlpool compensation tank. This was corrected during the inspection.
Recommendation: Ensure that air gaps are at least twice the diameter of the delivery fixture opening and a minimum of 3 cm (1 inch).

Violation: There was no backflow prevention device on the potable water line supplying the coffee machine.
Recommendation: Ensure that a continuous pressure-type backflow preventer is installed when a valve is located downstream from the backflow preventer.

Violation: On the cruise from 9/7 to 9/14/08, there was one crew member with gastrointestinal illness. The two cabin mates of the ill crew member were only followed up for 24 hours.
Recommendation: Ensure that medical staff conduct daily verbal interviews with asymptomatic crew until 48 hours after onset of the ill crew members symptoms.

Violation: There were no side sneeze guards at the action station.
Recommendation: Ensure that side sneeze guards are installed where food is placed within one meter of the end of the buffet where consumers line up for service.

Violation: There were slotted fasteners on the coils for the fryer.
Recommendation: Replace the slotted fasteners with smooth, low-profile, non-corroding fasteners.

Violation: The bulkhead panels and the top panels inside the technical compartment for the bain marie were loose.
Recommendation: Ensure that nonfood-contact surfaces of equipment and utensils are designed and constructed to be durable and to retain their characteristic qualities under normal use conditions.

Violation: There was exposed insulation inside the technical compartment for the bain marie.
Recommendation: Ensure that nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Violation: The final rinse curtain for the dishwash machine was soiled.
Recommendation: Ensure that warewashing machines; the compartments of sinks, basins, or other receptacles used for washing and rinsing equipment, utensils, or raw foods, or laundering wiping cloths; and drain boards are cleaned: (1) Before use; (2) Throughout the day at a frequency necessary to prevent recontamination of equipment and utensils and to ensure that the equipment performs its intended function; and (3) If used, at least every 24 hours.

Violation: A staff member was observed drying the utensils with a cloth.
Recommendation: After cleaning and sanitizing, ensure that equipment and utensils are air-dried or adequately drained before contact with food.

Violation: In the walk-in refrigeration unit inside the fish preparation, there was a bent horizontal profile strip on the lower edge of the bulkhead to the right of the entrance.
