    Cruise Ship's Corrective Action Statement

Item No.: 03
Site: Potable Water-Bunkering Charts
Violation: On 07/08, 08/4, 11, 12, 19, 23, 26, 09/2 and 09/27, the ship bunkered potable water at less than 2 ppm for the entire bunkering period.
Recommendation: Ensure that potable water is continuously halogenated to at least 2.0 mg/L (ppm) free residual halogen at the time of bunkering or production with an automatic halogenation device that is controlled by a flow meter.

Site: Potable Water-Bunkering Charts
Violation: On 07/4, 7, 14 and 09/04, the initial chlorine residual for bunkering was less than 2 ppm. It took more than 1/2 an hour to reach 2 ppm during bunkering.
Recommendation: Ensure the chlorine residual level reaches 2 ppm within the first half hour of bunkering.

Site: Potable Water-Hair Salon
Violation: There was no backflow prevention device installed on the hot potable water line for the hair sink.
Recommendation: Ensure that a continuous pressure-type backflow preventer is installed when a valve is located downstream from the backflow preventer.

Violation: There was no backflow prevention device installed on the potable water line with a hose connection.
Recommendation: Ensure that backflow preventers are installed when air gaps are impractical or when water under pressure is required.

Site: Potable Water-Engine Room
Violation: The chlorine injection point for potable water bunkering was less than 3 meters from the chlorine sample point.
Recommendation: Ensure that water samples for halogen testing are obtained from a sample cock located on the bunker or production water line at least 3 m (10 feet) after the halogen injection point and before the storage tank.

Site: Potable Water-Port Midship Bunker Station
Violation: The cap for the potable water filling line was able to touch the deck when hanging free.
Recommendation: Ensure that the potable water filling line has a screw cap or plug fastened by a non-corroding chain to an adjacent bulkhead or surface in such a manner that the cap or plug does not touch the deck when hanging free. The connections for the hose attachments shall be unique and fit only potable water hoses.

Site: Galley-
Violation: The backflow prevention device in the hood cleaning system was dripping at the intermediate vent. A new backflow prevention device was on order.
Recommendation: Ensure that backflow preventers are maintained in good repair.

Violation: There was a non-continuous pressure backflow prevention device installed on the potable water line to the coffee machine.
Recommendation: Ensure that a continuous pressure-type backflow preventer is installed when a valve is located downstream from the backflow preventer.

Site: Whirlpool/Spa-Lido Midship Whirlpool
Violation: A few of the suction covers were not anti-entrapment covers on both the starboard and port whirlpools. Per staff, they ordered the wrong ones and the new ones were on order.
Recommendation: Ensure that anti-entrapment drain covers or other drains that prevent entrapment hazards as specified in U.S. Consumer Product Safety Publication 363-009801 (dual drains/channel drains) are provided on swimming pools and whirlpool spas. Ensure that drain covers have either the plumbing/engineering approving organization stamp and flow rate affixed to the cover, or that a letter is maintained aboard the vessel certifying that the cover meets the safety requirements outlined in AMSE/ANSI A112.19.8M or an equivalent standard.

Site: Swimming Pool-Slide Pool
Violation: The shepherd's hook and the approved floatation device were not located in a prominent location.
Recommendation: Ensure that an easy-access shepherd's hook and approved floatation device (with a length of line 1/2 the pool width or 16 m (50 feet)) is provided at a prominent location near each public swimming pool.

Site: Whirlpool/Spa-All Whirlpools
Violation: The stamp on the suction covers stated "important this side down". The suction covers were incorrectly installed with the stamp on top.
Recommendation: Ensure suction covers are installed according to manufacturer's instructions.

Violation: The approved floatation device was not located in a prominent location.
Recommendation: Ensure that an easy-access shepherd's hook and approved floatation device (with a length of line 1/2 the pool width or 16 m (50 feet)) is provided at a prominent location near each public swimming pool.

Site: Buffet-Beverage Stations
Violation: The milk containers at the beverage stations had blue lids at 0945. According to the time as a public health control plan, the containers should have been replaced with containers with black lids at 0800. The containers were replaced.
Recommendation: Ensure the milk containers are replaced according to the time as a public health control plan.

Site: Buffet-New York Deli
Violation: There were side-dish items placed under a sneeze guard for passenger self-service. The sneeze guard was adjusted too high to protect the food and a container of chips were placed outside the sneeze guard. This was corrected and the buffet was not yet open for passengers.
Recommendation: Ensure that food on display is protected from contamination by the use of packaging; counter, service line, or salad bar food guards; display cases; or other effective means.

Site: Food Service General-Slicers
Violation: There was soft sealant around the back plate of several slicers. The sealant was food-grade sealant.
Recommendation: Ensure the sealant is hard or semi-hard.

Site: Buffet-Forward Starboard Grill Pantry
Violation: There was excess electrical cable under the hot holding unit. The excess cable was draped on the counter under the unit.
Recommendation: Remove the excess electrical cable.

Violation: There was an unsealed pipe penetration in the back of the technical compartment below the port side induction cooker.
Recommendation: Seal the pipe penetration.

Site: Galley-Dishwash
Violation: The temperature gauge on the flight-type dishwash machine was not accurate. The temperature indicated 162°F and the plate temperature was measured at 163°F.
Recommendation: Ensure that warewashing equipment is maintained in good repair and proper adjustment including: (1) Being maintained in a state of repair and condition that meets the standards of the materials, design, and construction of these guidelines; and (2) Maintaining water pressure, and water temperature measuring devices in good repair and ensuring their accuracy within the intended range of use.

Violation: The turning handle for the center tiling pan was soiled on the back with dried food debris.
Recommendation: Ensure that nonfood-contact surfaces of equipment are kept free of an accumulation of dust, dirt, food residue, and other debris.

Violation: There was a large open void on the door frame where the locking pin inserted allowing access for insects and making cleaning difficult.
Recommendation: Seal the opening.

Violation: The pipe penetration for the new sprinkler system in the upper right corner above the entrance was not sealed.

Violation: To the right of the entrance, there was a bulkhead panel that had buckled, creating a vertical gap near the lower edge of the bulkhead.
Recommendation: Ensure that decks, bulkheads, and deckheads in food preparation, warewashing, pantries, and storage areas are constructed and maintained for easy cleaning.

Violation: There was missing and peeling sealant along the backsplash/bulkhead juncture.
Recommendation: Replace the sealant.

Violation: There was cracked, recessed and missing grout throughout the galley, especially at drain and gutterway junctures with deck tiles.

Violation: Each of the painted steel door frames had holes for the locking pins. The holes did not have recessed nipples and were open to the door frame void.
Recommendation: Seal the holes in the door frames.

Violation: The deck/cabinet junctures in the lido buffet area and deck/bulkhead junctures in the dining room waiter stations were not coved.
Recommendation: Ensure that bulkhead and deck junctures are coved (including galleys, pantries, deck/counter junctures at buffets, bars, waiter stations, and dining room work counters).

Violation: There were excess electrical wires on the deck in the technical compartment below the computer. The deck under the wires was soiled.
Recommendation: Ensure that decks, bulkheads, deckheads, and attached equipment in food preparation, warewashing, pantries, and storage areas, are cleaned as often as necessary.

Violation: There were excess electrical wires on the deck in the technical compartment below the computer. The deck under the wires was soiled.
Recommendation: Ensure that exposed utility service lines and pipes, including lines for fire detection and protection systems, are installed so they do not obstruct or prevent cleaning.

Recommendation: Replace the overhead light.

Violation: There was heavy condensation in the hood above the dishwash unit.
Recommendation: Ensure that ventilation hood systems and devices are sufficient in number and capacity to prevent grease or condensation from collecting on bulkheads and deckheads.
