Item No.: 02
Site: Medical-Shipping Containers and Bacterial Transport Media
Violation: The ship does not have a proper shipping container and labels to transport clinical specimens under the requirements of the International Air Transport Association (IATA) and US. Department of Transportation (DOT). The ship does not have the proper shipping labels and refrigerant packs. The ship does not have the required 10 bacterial specimen collection containers.
Recommendation: Ensure that at least 10 clinical specimen collection containers for both viral and bacteriological agents (10 each), as well as the proper shipping containers and labels for same, are maintained by the medical staff.

Violation: The ship does not have a document to capture the 72-hour crew GI history for new and returning crew members. The crew physician is evaluating these individuals and those that report "yes" to diarrhea and vomiting are isolated depending on the respective job categories.
Recommendation: Ensure that follow-up with and approval by designated medical personnel is completed before returning crew (food and non-food) to work.

Recommendation: Ensure that reportable cases of gastrointestinal illness include crew members with a symptom onset time of up to 3 days before boarding the vessel. Maintain documentaion of the 3 day assessment, for each crew member WITH SYMPTOMS, on the vessel and make available for review during inspections.

Violation: The ship does not have a document to capture the 72-hour crew GI history for new and returning crew members. The crew physician is evaluating these individuals and those that report "yes" to diarrhea and vomiting are isolated depending on the respective job categories.
Recommendation: Ensure that symptomatic food employees meeting the case definition for AGE are isolated in cabin or designated, restricted area until symptom-free for a minimum of 48 hours.

Recommendation: Ensure that symptomatic non-food employees meeting the case definition for AGE are isolated in cabin or designated, restricted area until symptom-free for a minimum of 24 hours.

Violation: The free halogen residual of the bunkered water was 0.42 ppm. There was no sample valve on the tank that the bunkered water was filling.
Recommendation: Ensure that potable water is continuously halogenated to at least 2.0 mg/L (ppm) free residual halogen at the time of bunkering or production with an automatic halogenation device that is controlled by a flow meter.

Violation: An automated flow meter controlled chlorine dosing system was not provided on the lines from the evaporator to the potable water tanks. The potable water tanks were batch chlorinated during production using the evaporator.
Recommendation: Ensure that potable water is continuously halogenated to at least 2.0 mg/L (ppm) free residual halogen at the time of bunkering or production with an automatic halogenation device that is controlled by a flow meter.

Recommendation: Ensure that all distribution water system halogenation devices are constructed and installed in accordance with recommended engineering practices.

Violation: An automated chlorine dosing system has not been installed on this vessel. Chlorine was being added directly to the tanks. The far point free chlorine residual was at the proper level during this inspection.
Recommendation: Ensure that at least one backup halogen pump is available with automatic switchover to maintain the free residual halogen in the event that the primary pump fails, an increase in demand occurs, or the low chlorine alarm sounds.

Violation: A sample was collected for an e. coli test for the water bunkered in New York on 07/13/2008 but this sample still had not been tested on 07/14/2008 and the engineer had the sample in his pocket.
Recommendation: Ensure that these records are maintained on the vessel for 12 months and available to the VSP for review during inspections.

Recommendation: Where available, ensure that the vessel has a copy of the most recent microbiologic report from each port before bunkering potable water to verify that the water meets potable standards.

Violation: There were six potable water tanks that shared a wall with the hull of the vessel. Per the tank schematics these tanks were 22BB, 21BB, 22SB, 21SB, 23BBA and 23SBA.
Recommendation: Ensure that potable water tanks do not share a common wall with the hull of the vessel or with tanks containing non-potable water or other liquids.

Violation: The reverse osmosis unit and the evaporator both were used to make potable water and technical water. There was no backflow prevention device installed on the lines to the non-potable water system (technical water or boiler water).
Recommendation: Provide an air gap or reduced presssure backflow prevention device on the technical water line to prevent contamination of the potable water system.

Additionally, there was no indication that the existing backflow prevention devices were inspected or tested.
Recommendation: Ensure that backflow prevention devices requiring testing, for example reduced pressure backflow preventer and double check valves with test cocks, are inspected and tested with a test kit at least annually and that the test results showing the pressure differences on both sides of the valves are maintained for each device.

Recommendation: The vessel shall provide a comprehensive cross- connection control program that provides safe connections to the potable water system through air gaps or appropriate backflow devices at the following locations, if present: (1) Potable water supply lines to swimming pools, whirlpool spas, hot tubs, bathtubs, showers, and similar facilities; (2) Photographic laboratory developing machines and utility sinks; (3) Beauty and barber shop spray-rinse hoses; (4) Potable water faucets where hoses are connected or can be connected by threaded or quick-connect outlets such as those serving tanks containing chlorine and other chemicals, and deck taps; (5) Garbage grinders and pulpers; (6) Mechanical warewashing machines; (7) Hospital and laundry equipment; (8) Air conditioning expansion tanks; (9) Boiler feed water tanks; (10) Fire systems; (11) Toilets; (12) Potable water, bilge, and sanitary pumps that require priming; (13) Freshwater or saltwater ballast systems; (14) Bilge or other waste water locations; (15) International shore connection; and (16) Any other connection between potable and non-potable water systems.

Violation: There were numerous potable water connections throughout the vessel that were not fitted with backflow prevention devices. These connections included the toilet connections for passenger cabins 7029, 7031, 7033, 7041, 7032 and 7030; the passenger shower hose connections; the wash hoses in the japanese bath room, the international shore connections in the forward and aft mooring stations; the hose connections on the bunker station filling lines; the crew beverage station juice machine; the officer's mess coffee machine; the engine work shop threaded hose connection; the main laundry washing machine water connections; the deck 5 passenger laundry; and the hose connection near the tilting kettle in the crew galley.

Recommendation: Ensure that documentation of any inspection, cleaning, disinfection (including concentration and contact time of disinfectant), and flushing is maintained for 12 months and is available to the VSP for review during inspections.

Recommendation: Ensure that interior coatings on potable water tanks are approved for potable water contact, and all manufacturer's recommendations for application, drying, or curing are followed. Ensure written documentaion for the coating used and recommendations followed are avaialble for review during inspections.

Recommendation: Ensure that potable water tanks are inspected, cleaned, and disinfected during dry docks and wet docks, or every 2 years, whichever is less.

Violation: The reduced pressure backflow prevention device on the high saline overboard line from the evaporator was heavily painted. It was clear that this device could not be tested in its present condition.
Recommendation: Ensure that this device can be tested or replace it.

Violation: The seawater sand filters prior to the reverse osmosis unit had a backwash line that was directly connected to an overboard line. There was no backflow prevention on this line. Additionally, it was not clear how many other lines were also connected to this overboard line.
Recommendation: Provide an air-gap at the end of the backwash line to the bilge.

Violation: The hose connection on the line from the reverse osmosis unit to the potable water system was not protected. While this line was prior to chlorination it was part of the potable water system.
Recommendation: Remove this hose connection.

Violation: The free chlorine sample point for the reverse osmosis unit was less than 3 meters after the injection point.
Recommendation: Ensure that water samples for halogen testing are obtained from a sample cock located on the bunker or production water line at least 3 m (10 feet) after the halogen injection point and before the storage tank.

Recommendation: Ensure that potable water piping and fittings are painted or stripped auxiliary blue, or in accordance with ISO 14726, at 5 m (15 feet) intervals on each side of partitions, decks, and bulkheads in all spaces, except where the decor would be marred by such markings.

Violation: The chains on the caps for the port bunker station filling lines were too long.
Recommendation: Ensure that the potable water filling line has a screw cap or plug fastened by a non-corroding chain to an adjacent bulkhead or surface in such a manner that the cap or plug does not touch the deck when hanging free. The connections for the hose attachments shall be unique and fit only potable water hoses.

Violation: The potable water hoses were not labeled "potable water only" at each connection end.
Recommendation: Ensure that potable water hoses are labeled for use with the words "potable water only" in letters at least 13mm (0.5 inch) at each connecting end.

Violation: The pool was said not to be in use and had not been in use since April 2008. This pool had a single drain that was not fitted with an anti-entrapment drain cover. Per the Staff Captain the halogen dosing, pH control and filtration systems were to be replaced prior to using this pool again.
Recommendation: Ensure that anti-entrapment drain covers or other drains that prevent entrapment hazards as specified in U.S. Consumer Product Safety Publication 363-009801 (dual drains/channel drains) are provided on swimming pools and whirlpool spas. Ensure that drain covers have either the plumbing/engineering approving organization stamp and flow rate affixed to the cover, or that a letter is maintained aboard the vessel certifying that the cover meets the safety requirements outlined in AMSE/ANSI A112.19.8M or an equivalent standard.

Violation: The ship's medical staff is not providing a personal hygiene/handwashing tip sheet to passengers diagnosed with gastrointestinal illness.

Violation: Several containers of milk were found at 53°F inside the reach-in refrigerator. One container of milk was tested and was found with an internal product temperature of 51.6°F. The containers of milk were placed into the blast chiller for rapid chilling.
Recommendation: Store milk at or below 5 °C (41°F).

Violation: The vessel did not have documentation on the freezing time and temperature for the cold smoked salmon served on the day of the inspection. Per the Food and Beverage Manager raw fish was not served on the vessel while it was in the United States.
Recommendation: Before service in ready-to-eat form, ensure that raw, raw-marinated, partially cooked, or marinated-partially cooked fish other than molluscan shellfish: (1) Are frozen throughout to a temperature of -20°C (-4°F) or below for 168 hours (7 days) in a freezer; or -35°C (-31°F) or below for 15 hours in a blast freezer.

Recommendation: Ensure that multiuse food-contact surfaces are: (1) Smooth; (2) Free of breaks, open seams >1 mm (1/32 inch), cracks, chips, inclusions, pits, and similar imperfections; (3) Free of sharp internal angles, corners, and crevices; (4) Finished to have smooth welds and joints; and (5) Accessible for cleaning and inspection by one of the following methods without being disassembled, by disassembling without the use of tools, or by easy disassembling with the use of handheld tools commonly available to maintenance and cleaning personnel such as screwdrivers, pliers, open-end wrenches, and Allen wrenches.

Violation: There were holes on the refrigeration unit doors where the locking mechanisms were missing. This was noted for refrigeration units 54B and 54 E. Additionally, there was tape on the front panels of refrigeration units 54E and 55 A-B. These conditions made cleaning difficult.
Recommendation: Seal/plug the holes. Remove the tap and repair the damage to the front panels of the refrigeration units.

Violation: The housing for the motor on the single compartment dishwasher was corroded, making cleaning difficult.
Recommendation: Ensure that nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning are constructed of a corrosion-resistant, nonabsorbent, and smooth material.
