Site: Medical-Questionaires
Violation: For the 3/24-31/2007 cruise the 72-hour questionnaire for a crew member was missing.
Recommendation: Ensure that questionnaires containing all of the data elements that appear in the questionnaire found in Annex 13.2 and detailing activities and meal locations for the 72 hours before illness onset are distributed to all passengers and crew members who are gastrointestinal illness cases. Ensure that the completed questionnaires are maintained alongside the gastrointestinal illness surveillance log.

Violation: The backflow protection device for the hood cleaning system was leaking. The container that should hold the cleanser was filled with water and standing water was noted on the deck due to this leak.
Recommendation: Ensure that backflow preventers are maintained in good repair.

Recommendation: Ensure that vent openings are not blocked.

Violation: There was no record of the monthly sand filter checks and sedimentation tests.
Recommendation: Ensure that the granular filters are opened at least monthly and examined for cracks, mounds, or holes in the filter media and a core sample of the filter media is inspected for excessive organic material accumulation using a recommended sedimentation method.
