BACKGROUND:

The current Indian Health Manual Part 3, Chapter 13, Maternal and Child Health, Transmittal Notice 85.6 was issued July 1, 1985.  The attached Appendix III is to replace page 43 contained in the document TN 85.6.  The attached Appendix III, page 43, contains guidelines to fill in the information requested.
