Trial Description

Purpose:

Drugs used in chemotherapy work in different ways to stop the growth of tumor cells, either by killing the cells or by stopping them from dividing. Giving more than one drug (combination chemotherapy) together with a peripheral stem cell transplant may allow more chemotherapy to be given so that more tumor cells are killed. It is not yet known which combination chemotherapy regimen is more effective when given before a peripheral stem cell transplant in treating supratentorial primitive neuroectodermal tumors or medulloblastoma.

This randomized phase III trial is studying two different combination chemotherapy regimens to compare how well they work when given before a peripheral stem cell transplant in treating young patients with newly diagnosed supratentorial primitive neuroectodermal tumors or high-risk medulloblastoma .

Eligibility:

Eligibility criteria include the following:

Final eligibility for a clinical trial is determined by the health professionals conducting the trial.

Treatment/Intervention:

Patients will be randomly assigned (have an equal chance of being placed) to one of two treatment groups.

Patients in group one will receive an infusion of vincristine on days 1, 8, and 15. They will also receive 1-hour infusions of etoposide on days 1-3 and cyclophosphamide on days 1 and 2, a 6-hour infusion of cisplatin on day 3, and G-CSF by infusion or by injection beginning on day 4 and continuing until blood counts return to normal. Treatment may repeat every 3 weeks for up to three courses

Patients in group two will receive an infusion of vincristine on days 1, 8, and 15. They will also receive a 4-hour infusion of methotrexate on day 1 and leucovorin by mouth or by infusion every 6 hours beginning on day 2 and continuing until the level of methotrexate in the blood is within a safe range. Once the level of methotrexate is within a safe range, patients will receive 1-hour infusions of etoposide once a day on days 4, 5, and 6 and cyclophosphamide once a day on days 4 and 5, followed by a 6-hour infusion of cisplatin on day 6. They will also receive G-CSF once a day beginning the day after the completion of chemotherapy and continuing until blood counts return to normal. Treatment may repeat every 3 weeks for up to three courses.

Some patients will then undergo surgery.

Beginning no more than 6 weeks after finishing chemotherapy or after recovering from surgery, patients in both groups will receive 2-hour infusions of carboplatin and thiotepa on days 1 and 2 and G-CSF by infusion or by injection beginning on day 54 and continuing until blood counts return to normal. They will also receive an infusion of peripheral stem cells on day 4. Treatment may repeat every 3 weeks for up to three courses.

Before beginning treatment, patients will undergo blood and tissue sample collection for laboratory studies.

Patients will undergo quality of life and neuropsychological evaluations before beginning treatment, after finishing the first three courses of chemotherapy, and at 1, 2, and 4 years after finishing treatment. After finishing treatment, patients will be evaluated periodically.
