Registration for Other Federal Agencies

Submit an agency training nomination or registration form through appropriate agency approving officials and/or training offices. The appropriate billing number and address must be indicated at the bottom of the form. Include the following information:

• Title, course number, date, location, and cost of the course
• Billing information: office name, address, and billing account number
• Name, office address, phone number and e-mail address of the nominee
• Position title and grade of the nominee

Fax a copy of the approved training nomination or registration form to 301-480-3197.

The NIH Training Center will email or fax notification to the students indicating the status of their request for training. Telephone reservations will not be accepted.
