WELCOME HOME!
We value your service and sacrifice, and the sacrifice of your families.
The VA Healthcare - VISN 4 Network is committed to support returned troops from Operations Enduring Freedom and Iraqi Freedom and to ensure that they have the health care and benefits they need.
Enrollment and Eligibility:
As a recently discharged veteran, it is very important to note that you have two years from your date of discharge to receive free health care for any condition that may be related to combat service. After the two years, a co-payment status will depend on whether your illness or injury is found to be service-connected.
There are several ways you may enroll for care:
• either by coming to a VA Medical Center or
Community Based clinic near you
• By using the 10-10EZ online enrollment form
• or by mailing or faxing the completed 10-10EZ form
to the nearest Medical Center or clinic
Veterans are eligible if they served on active duty in a theater of combat operations during a period of war after the Gulf War or in combat against a "hostile" force during a period of hostilities after November 11, 1998 and have been discharged under other than dishonorable conditions. If you are in the National Guard and Reserve component you are eligible as a combat veteran, if you were ordered to active duty by a federal declaration, served the full period of which you were called or ordered to active duty, and have separated from active military service under other than dishonorable conditions.
Benefits: This benefit covers all illnesses and injuries, except those clearly unrelated to military service, free of charge for a period of two years from the date of discharge. Co-payments may be charged for treatment of non-combat related conditions (common cold, injuries from accidents that occurred after discharge or disorders that existed before joining the military).
If a veteran did not receive a pre-separation dental exam from the military, then the veteran is eligible to receive a dental exam from the VA within 90 days of their discharge from active duty.
