I noticed the Office of Management and Budget (OMB) authorization date for the Form 561 is 5/31/08. Is this the latest version of the form? I wanted to make sure there was not another version of the form I should be filling out instead. |

The version of the Form 561 that is on this Web site is the correct form to fill out. We are in the process of renewing our OMB authorization to collect the Form 561 data and the expiration date will change at the end of that process. |
