The attached file--an Item Approval Request List (IARL) form-- was created using MSWord 2003. Please provide complete information on the IARL for boxes "A" (customer/researcher information), "B1" (control and reel numbers--use the number that appears in the "Local Identifier field"), "B2" (subject), and "B3" (order copy format, this is the copy you want delivered to you). Each reel must be listed separately. If you want to order more than 10 items, you will need to complete a continuation sheet. Please be certain to provide your name and the number of pages for your complete order at the top of the continuation sheet. You may order up to 23 reels per order and you may submit two IARL per week. Please return completed IARL as an e-mail attachment. You may make multiple copies of this form.
