This is the first step in our effort to simplify, standardize, and make communication with us easier. In conjunction with the recent FAA streamlining of the application process, the FAA is providing this optional spreadsheet for commercial service and primary nonhub airports to submit their PFC data.

The Attachment H to Form 5500-1 (formerly 'Nonhub Notice of Intent Worksheet') (MS Excel) can be downloaded and data entered. When you save this data, please use the 3 letter LOCID for your airport and the application number in the file name. For example "XYZ 04 PFC.xls". This will help us easily identify, sort, and file this information.

There are 3 workbooks in the spreadsheet. It is designed so that you only have to enter recurring data (such as project name) once, and will automatically update the other workbooks. It is also designed so that the first workbook contains all the information needed to be your public notice. Plus, you should be able to post that page on your website to meet Part 158 requirements.

There are comments that explain data needs. By using the mouse to place the cursor over the column header, you can see the comment. The fields which will accept data are shaded. The others cells are locked, and may not be changed.

The information currently in the spreadsheet is provided as an example, and will not be applicable in all cases.

Finally, the worksheet needs to be accompanied with a signed FAA Form 5500.1. For your convenience, you may use one of the following methods to transmit the necessary PFC data to FAA, either:
(1) email the worksheet and fax Form 5500.1; or (2) use the post office to send both documents.

As a reminder, please do not submit the notice of intent before the public notice period and the carrier notice/consultation period has ended.
