OpScan Form Request
For people who need to obtain OpScan sheets for classroom training, please fill out the Independent Study Program OpScan Request Form below. Please allow four weeks for delivery.
When you complete the OpScan form, please leave them unfolded, and return them to the address on the back of each form. For paper exam test submissions, the exams are scored and certificates or failure letters are mailed within 5 business days. OR, tests can be submitted online with results in one business day. Please choose only one option for test submission.
