Referral Process



National Referral Program - Referral process for veterans with difficult to diagnose health problems

Healthcare providers interested in making referrals to the War Related Illness and Injury Study Center (WRIISC) National Referral Program should contact Ms. Helen Malaskiewicz, the Environmental Health Coordinator at VA Central Office, at 202-273-8463. A prescreen referral application (Form 10-0417) must be completed and sent to Ms. Malaskewicz by fax to 202-273-9562. When it is confirmed that a veteran is eligible for a WRIISC referral, Ms. Malaskewicz will ask the referring physician to complete the WRIISC clinical evaluation referral template in CPRS.

Once Ms. Malaskiewicz receives the completed CPRS template, she will triage the veteran to either the East Orange, NJ, or the Washington, DC, WRIISC based on the veteran’s geographic location. When the NJ WRIISC receives a referral application, the clinical team reviews the application and determines if the veteran meets the WRIISC eligibility criteria and is likely to benefit from a WRIISC evaluation. When a veteran is approved for a WRIISC clinical evaluation, NJ WRIISC staff will schedule an evaluation date and coordinate the visit.

Providers in the VA NJ HCS only can refer veterans with difficult to diagnose health problems to the NJ WRIISC by submitting a WRIISC consult in the local CPRS system. The process and eligibility criteria are the same as described above.

Referral Process for OEF/OIF veterans

OEF/OIF veterans who are interested in a WRIISC clinical evaluation should contact us at 1-800-248-8005. OEF/OIF veterans who would like to come to the center for a WRIISC clinical evaluation who have never received care in the VA NJ HCS will be asked to register in the VA NJ HCS by completing a 10-10EZ Form. Veterans will be instructed to bring a completed 10-10EZ form and a copy of an original DD214 to the Eligibility Office in the East Orange VA Medical Center (room A-277) to establish enrollment. NJ WRIISC staff will schedule OEF/OIF veterans for an appointment once registered in the VA NJ HCS and will assist in connecting veterans to other health care resources within the VA.

Healthcare providers interested in making a referral to the WRIISC OEF/OIF clinic may submit an electronic WRIISC consult in CPRS. Once the consult is received, NJ WRIISC staff will contact the veteran to schedule an appointment.
