Filling out Forms

Note: Forms MV-1 and MV-4ST are not available online. These forms must be completed and submitted by an authorized agent of the Department. You will need to contact a notary or dealer for assistance with the completion and submission of these forms.

Please remember to include a check or money order with all completed forms for transactions which require fees. Please note: the Department will charge a fee of $30.00 or more for each check returned to the Department as uncollectible.
