Completing the Form No. 556

No. 556 [DOC] | [RTF] is provided for the use of applicants submitting a self-certification or applying for Commission certification of QF status. Complete the information for items 1 to 15, as applicable. For items which are not applicable, state “N/A.” If the reason the item is not applicable is not obvious, provide details. Some entire sections of the Form No. 556 may be not applicable to your filing, and you may indicate as such. In particular, the following sections should not be completed, depending on your facility type:
- Applicants for certification of a small power production
facility should not complete Part C.
- Applicants for certification of a topping-cycle cogeneration
facility should not complete Part B or Item 14.
- Applicants for certification of a bottoming-cycle cogeneration
facility should not complete Part B, Item 12 or Item 13.
- Some applicants for certification of a cogeneration facility
will not need to complete Item 15.
Applicants for recertification do not need to repeat information in the Form No. 556 that they have provided to the Commission in previous filings. However, applicants should respond to each item in the Form No. 556 and either:
- Provide the current information for that item;
- State that the item is not applicable (N/A); or
- Incorporate the data provided for that item by reference to the date that the data for that item were previously provided. For example, an applicant that has already provided information for item 3b in their original certification or a subsequent recertification may state in their current filing that "Information for item 3b has not changed. This information was originally provided to the Commission in a Form No. 556 filed in this Docket No. on September 2, 2000."
All applicants are required to sign their Form No. 556 at Item 1d. A signature on a filing shall constitute a certificate that (1) the signer has read the filing and knows its contents; (2) the contents are true as stated, to the best knowledge and belief of the signer; and (3) the signer possesses full power and authority to sign the filing. A person submitting a self-certification electronically via eFiling may use typed characters representing their name to show that the person has signed the document. See 18 C.F.R. § 385.2005 .
