Incorrect Telephone Number On Id Cards
On October 01, 2008 ID cards with an incorrect telephone number were
sent out to AHCCCS members of University Family Care Health Plan. We are making
corrections. The correct number is 1-800-582-8686.
We apologize for the inconvenience. Services will not be denied or delayed due
to this error. If you received a card with the incorrect number you will receive
a replacement card.
All other health plan information is correct.

As allowed by CMS,
late last spring AHCCCS elected to
deploy an NPI contingency plan which
allowed for the continued dual use of
the NPI for service/rendering providers
through February 29, 2008 dates of
processing. This contingency period has
now expired and
AHCCCS will enforce appropriate full
compliance with the NPI provisions for
processing on and after 3/1/2008 for
service/rendering providers. For
