VA Hudson Valley Health Care System Homeless Programs

The VA Hudson Valley Health Care System (HVHCS) provides services to homeless veterans in the lower Hudson Valley and the New York metropolitan areas. In addition to a full range of inpatient, residential and outpatient services, there are program components specifically for homeless veterans. These include:

Health Care for Homeless Veterans Program (HCHV)

The HCHV Program provides outreach and case management services to veterans in homeless shelters and drop-in centers. The outreach staff, which includes graduates of HVHCS programs, provides counseling and linkage with VA medical and psychiatric appointments. A community residential contract program is available to help veterans leave the streets and prepare for treatment programs. HCHV works closely with community agencies, actively partnering with several of them in the VA Homeless Providers Grant & Per Diem Program, which provides transitional housing.

Domiciliary Care for Homeless Veterans Program (DCHV)

The Domiciliary provides rehabilitation services to homeless veterans in a 60 bed residential setting. This is a structured 4 month program which addresses the range of conditions which both contribute to and result from homelessness: substance abuse; psychiatric instability; lack of housing; unemployment; lack of social and life management skills; and lack of community support systems. Based ion the veteran's needs, individual and group counseling, vocational rehabilitation and medical and psychiatric care are provided. Skills training groups focus on anger management, relapse prevention, money management and cooking on a budget. The Dom is a JCAHO accredited program.

Vocational Rehabilitation Therapy Program (VRT)

In this program, veterans have the opportunity to explore their vocational skills and interests, and to develop and obtain employment and/or educational opportunities. Assessment, testing, counseling, planning and placement are essential parts of the comprehensive vocational treatment plan. Veterans develop resumes, practice job interviewing skills, and receive placement services from a full-time onsite NYS Department of Labor representative. Supervised onsite Incentive Therapy and Compensated Work Therapy assignments enable veterans to develop work readiness skills and earn money to secure community housing. Basic computer training is provided for all veterans in VRT. VRT is accredited by CARF.

Supportive Housing Program (SHP)

The SHP provides affordable transitional housing for homeless veterans discharged from HVHCS programs. The SHP case manager assists veterans in setting up their homes; developing budgets; addressing time management; utilizing their community networks; and successfully balancing their needs and responsibilities.
