Subject:  Campus Based Policy Memorandum 2004-1

Please forward this information to the financial aid administrators and fiscal officers responsible for administering the Department of Health and Human Services, Division of Health Careers Diversity and Development (DHCDD)/ Campus-Based Scholarships for Disadvantaged Students (SDS) Program.

This policy memorandum provides an update on the reporting requirements for the SDS Program administered by DHCDD. 

AY 2005-2006 SDS Application Reporting Requirement: 

Primacy Care (PC) information required under Section 737(c) will be collected on the SDS Application beginning with AY 2005-06.  Schools should determine how many graduates for the reporting year are practicing in PC and then determine how many of those graduates received SDS.

The primary care disciplines included are allopathic medicine, osteopathic medicine, dentistry, graduate nursing (mid wifery and nurse practitioner), and physician assistants.
