Transformacion Para Salud will improve health care outcomes for vulnerable individuals in Lubbock county.

Using a cadre of certified promotores with community health worker certification as patient navigators, the project aims to coordinate comprehensive health services for patients in need of chronic disease care and management for the following health conditions:

  • cancer
  • diabetes
  • hypertension
  • obesity
  • asthma

The nexus emanates from the Larry Combest Community Health and Wellness Center, a 501(c)(3) primary care center owned and operated by the School of Nursing at the Texas Tech University Health Sciences Center.

The target population is economically and medically vulnerable patients with the identified health conditions.

Using four trained promotores, Transformacion Para Salud aims to serve a maximum of 250 patients currently enrolled in the Larry Combest Community Health and Wellness Center chronic disease management programs over the two-year project period.

The Transformacion Para Salud/Transformation for Health conceptual framework developed in the school of nursing will be a foundation for the enhanced promotores training curriculum. The chronic disease management model will be layered onto the curriculum that provide the knowledge base required for the promotores practice.

During the second year, the project team will investigate the potential for disseminating the promotores training program to surrounding rural counties.

The team will assess need for community health worker outreach in selected rural communities and issue reports to stakeholders within those communities to determine program priorities.

When indicated, the team will work with those stakeholders to develop the training program for those communities.
