簡介
儘管侵入性宮頸癌的發病率在過去50年裏急速下降,然而該疾病每年仍有近10000個新病例並造成3700例死亡。考慮到早期檢出的療效,侵入性宮頸癌不應再是一項健康威脅。然而,全國性研究顯示許多女性,尤其是年老女性,並不接受宮頸塗片(Pap)檢查。

方法
在此補充研究中,我們分析了來自全國健康訪談調查(National Health Interview Survey)的資料,該調查主要研究年滿55歲女性的篩查關聯性,這個年齡組女性的侵入性宮頸癌發病率逐步上升而接受宮頸塗片的比率卻下降。為了更多瞭解基礎觀點,我們向25位很少或從來都沒有進行篩查的女性,詢問其不接受宮頸塗片檢查決定的根本原因和情況。

結果
定量性資料提示宮頸塗片檢查的使用情況與以下因素之間有關聯:人口統計因素(已婚、年輕以及居住在近郊或郊區)以及使用預防性照護的途徑(接受乳腺X光攝影檢查、擁有常規健康照護來源以及可以聯絡婦産科醫生)。提供定性資料的參與者重復體現了此預防性服務使用不足的論題,尤其是社會聯繫較弱的女性,即較年老以及居住在鄉村地區的女性。匱乏健康照護專業人員以及缺少連續照護和私密性造成了預防性照護不佳。

結論
出現惡性循環:由於健康和經濟需求相互競爭以及沒有足夠的資源來獲取照護,許多女性拒絕獲取預防性照護。當此類女性前往醫生診所就診時,他們感覺受到了醫護人員的斥責,造成他們疏遠並不願接受未來的預防性照護。
