Veterans can apply for VA health care enrollment by completing VA Form 10-10EZ, Application for Health Benefits. The application form can be obtained by visiting, calling or writing any VA health care facility or veteran's benefits office. Forms can also be accessed from our website. Completed applications must be signed and dated and may be submitted in person to one of our Southeast Louisiana Veterans Health Care System outpatient clinics or mailed to:
