The Outreach Sourcebook (Volume 4, September 1998)

This report discusses the experiences of 81 projects in 42 states and two territories that received Rural Health Outreach Demonstration Grants. It describes the successes this group of Grantees has had creating lasting improvement to health care in their communities, strengthening the communication networks between providers, improving referral patterns, and enhancing continuity of care.
