Annette Kowal, Chair
Ms. Kowal currently works as the executive director of the Colorado Primary Care Association.  In this capacity she represents over 105 community health center sites and 400,000 underserved, low-income Coloradans.  She also negotiates health care policies and rates with providers, clients, legislators and other state, federal and local agencies.  She manages the recruitment and retention activities for 54 counties and she successfully developed and implemented site specific clinical continuous quality improvement systems. Ms. Kowal’s 16 years of experience in health care service to underserved communities and work with health centers make her an excellent chair for the council. 

Gary Batton
Mr. Batton currently serves as the executive health director of the Choctaw Nation of Oklahoma.  He heads a rural hospital that serves both Native Americans and non-Natives.  The program headed by Mr. Batton serves 44,000 patients across 11 counties of Southeastern Oklahoma and provides a full scope of services including primary care and mental and behavioral health services.  His role includes the management of 722 administrative and clinical employees. Mr. Batton became the deputy director of the Choctaw Nation Housing Authority where he oversaw all operations of the Housing Authority.  Areas of operation included staffing, new construction, finance, and purchasing.  He also developed and lead programs addressing the elimination of drugs in public housing and Family 6 investment programs.  Mr. Batton’s experience with recruitment and retention issues for sites and understanding of Native American health issues benefit the Council.

Byron J. Crouse, MD FAAFP
Dr. Byron Crouse began his medical career by practicing in a town of 1600 in northwest Wisconsin, and then decided to move to an educational setting.  For the last twenty years he has served as an Assistant Residency Director, Medical School Department Chairman and Associate Dean promoting rural health care.  As an educator, Dr. Crouse has been involved with supporting and developing a number of programs designed to nurture rural interests and prepare future physicians, pharmacists and nurses for rural practice.  In addition to his role at the University of Wisconsin School of Medicine and Public Health, Dr. Crouse is responsible for the Wisconsin Office of Rural Health and the Wisconsin AHEC Program.  He is also currently developing the Wisconsin Academy for Rural Medicine (WARM) in hopes of preparing more physicians of all specialties for practice in rural WI.

Delisa Culpepper, M.P.H., C.H.E.S., R.D.H.
Ms. Culpepper has been the Chief Operating Officer of the Alaska Mental Health Trust Authority since 2001. In this role, she coordinates strategic planning, communications and advocacy activities in collaboration with statutory partners. Immediately prior to this position, she worked as an adjunct professor at the University of Alaska at Anchorage School of Nursing teaching courses on program evaluation and grant writing. She practiced dental hygiene in a private practice from 1976-1980 and served as a supervisory community health dental hygienist for the DENTAC Command for the United States Army Alaska from 1980-1984. In this capacity, she created and managed the Alaska Dental Commands Community Dental Health Program and dental readiness program for active duty.

Cherae M. Farmer-Dixon, D.D.S., M.P.H.
Dr. Cherae Farmer-Dixon is the Associate Dean for Student Affairs in the School of Dentistry and Associate Professor in the Department of Dental Public Health at Meharry Medical College.  She also serves as Chairperson of the Dental Admissions Committee and the Dental Director of the Health Careers Opportunity Program (HCOP).  Dr. Farmer-Dixon has been actively involved in research activities that have dealt with oral health disparities, caries in low-income children, community outreach and intervention, and addressing minority dental school enrollment.  Her research endeavors have lead to presentations and publications at local, national and international meetings.  She has also received numerous awards from local, state, and national organizations as well as from her students. 

Corey J. Hebert, M.D.
Dr. Hebert is alum of the National Health Service Corps Scholarship (NHSC) Program and continues to serve the underserved in New Orleans, Louisiana. His educational accomplishments include a medical degree from Meharry Medical College and residency in pediatrics at Tulane University Medical Center.  Upon completing his education and training, Dr. Hebert served his NHSC obligation at the Excelth Community Health Center, serving as the director of Pediatrics to three sites.  Dr. Hebert continues his strong support for the NHSC programs by speaking to NHSC scholarship recipients.  In addition to his work at the Children’s Medical Clinic, Dr. Hebert owns and operates Hebert Medical Consulting, conducting FDA regulated clinical trials.  The council values Dr. Hebert’s experience through his completion of the NHSC program. 

Theresa V. Horvath, PA-C, MPH
Ms. Horvath currently is the Director of the Physician Assistant Program and Assistant Professor at Mercy College in The Bronx, NY.  She has developed and administers a new dual master’s degree program in Physician Assistant Studies.  Ms. Horvath has a strong history in community medicine, as Director of Community Medicine at The Presbyterian Hospital in the city of New York.  In addition, she has shown dedication to primary care through service to medical students and residents in urban clinical family medicine.  Furthermore, Ms. Horvath frequently presents at national conferences on topics such as administration of a physician assistant program and the importance of patient care in family medicine.

Donna E. Johnson, M.S.W., L.I.C.S.W
Donna Johnson is currently the Director of the Division of Primary Care and Health Access in the Massachusetts Department of Public Health (MDPH). The programs within this Division focus on increasing the availability of and access to comprehensive, quality primary care and critical support services for adults, adolescents and children across the Commonwealth. Ms. Johnson oversees maternal, child and adolescent primary care services within community health centers; Family Planning; School Health and School-Based Health Centers; Office of Rural Health; Office of Oral Health; Primary Care Office; and the safety net managed care program, Center Care. The Primary Care Office coordinates the State Loan Repayment Program, J-1 Visa Program, National Health Services Corps Program, health professional shortage designations and several new work force efforts addressing the nursing shortage and obstetrics access issues.  Ms. Johnson came to the Department with 20 years social work experience, with particular expertise in hospital-based MCH inter-disciplinary program development and implementation, community psychiatry, and child welfare policy and advocacy activities. 

Nycole Campbell-Lewis, Ph.D., LMSW
Dr. Nycole Campbell-Lewis has had extensive experience in a wide variety of health care administration tasks.  As a former Presidential Management Intern, Campbell-Lewis has extensive experience working in and around the federal government.  She used this work as the Executive Director of the Delta Health Alliance Foundation, where she advocated on a national, state and local level for policy change and development of health policy.  Campbell-Lewis currently serves as the Deputy Administrator/Chief of Staff in the Mississippi Governor Office of Medicaid.  In both roles, she is able to advocate for the underserved to receive the best possible care.

Kevin A. Lewis
As the Executive Director of the Maine Primary Care Association, Mr. Lewis works daily to advocate for policies and programs that improve health care access to the underserved.  In this role, Mr. Lewis also is able to promote primary care through Maine’s community health centers by representing them at local, state and federal functions.  Previous to his role at the Maine Primary Care Association, Mr. Lewis served at the Maine Hospital Association, and the Department of Health and Family Services of Wisconsin.  Mr. Lewis has a vast amount of experience dealing with legislative concerns of special interest groups as their chief lobbyist in budget matters.  In addition, Mr. Lewis has presented on issues such as access to cancer care in rural populations, and expanding overall access to quality care at numerous conferences. 

Jane Stevenson Martin, Ph.D., N.P.-C, A.P.R.N., B.C., F.N.P.
Dr. Martin is currently Assistant Dean of Graduate Studies at the Ida V. Moffett School of Nursing at Samford University in Birmingham, Alabama.  As Assistant Dean, Dr. Martin continually works with many graduate nursing students.  Dr. Marin has spent her entire career in nursing with a focus on the underserved in Alabama and other parts of the United States.  She uses her role as an educator to provide opportunities for students to work with the underserved.  For the past six summers, Dr. Martin has spent her time with students providing primary care services for migrant farm workers and their family members in Chandler Mountain, Alabama.  Dr. Martin continually avails herself to students who share her similar passion of service to the underserved, serving as both a mentor to these students and an Ambassador to the National Health Service Corps (NHSC). 

Stephen McKernan, M.A., C.P.A.
Mr. McKernan currently serves as the Chief Executive Officer of the University of New Mexico Health Sciences Center where he created a strategic plan for the Hospitals and its medical staff that emphasizes the development of the Children’s Hospital, the Cancer Research and Treatment Center, Neuroscience Center and Level I Trauma Center. Prior to his role as CEO, he served as the Executive Director of the University Of New Mexico School Of Medicine managing the day-to-day operations of a 440 physician, multi-specialty group practice. This is just one of his many significant accomplishments as CEO. Mr. McKernan has served as a member on several Boards, which includes the New Mexico Hospital Association, TriCore Reference Laboratory and the National Association of Public Hospitals, among others.  His background in hospital systems bring a unique perspective to the council. 

Tam T. Nguyen, M.D.
Dr. Nguyen is a NHSC Scholar currently in his first year of residency. Dr. Nguyen has shown continued dedication to NHSC throughout his scholarship by speaking at various NHSC conferences, as well as continually advocating the program to other students.  In addition to being a NHSC scholar, Dr. Nguyen served in leadership positions throughout his training at the American Medical Association, the American Academy of Family Physicians, and the American Medical Student Association.  Dr. Nguyen has also furthered his education through research during his training.  He has also served the underserved during training as a volunteer at the YWCA, the Bethesda Mission, Habitat for Humanity and LionCare, volunteer at a student-run medical clinic for homeless men. 

Luis F. Padilla, MD
Dr. Padilla, is currently serving his obligation for his NHSC Scholarship at the Upper Cardozo site of Unity Health Care as both physician and Associate Medical Director.  Padilla, who was the first in his family to attend and graduate from a university received his M.D. at Wake Forest University and completed his Family Medicine residency at Brown University.  In addition, Dr. Padilla has completed course work in public health and a fellowship in the DHHS Office of Minority Health.  Through this experience, he became more aware and committed to the care of the underserved and minority populations specifically.   Dr. Padilla became a NHSC Scholar at the end of his first year of medical school, when he knew he wanted to pursue a career as a primary care physician and particularly work with underserved communities.   His residency was linked with a FQHC in Rhode Island where he learned of the issues facing underserved communities and the challenges faced by FQHC’s in providing care and services.  Currently serving out his NHSC commitment at the Upper Cardozo site of Unity Health Care, Dr. Padilla actively sought out this position because of its location, mission, commitment to residents of the District, and the large number of immigrant patients it served.  In addition, he and his wife live within this community in order to benefit from and better understand community challenges as a part of the community he cares for.  Dr. Padilla can bring to the council a perspective based on experience as a current scholar at a FQHC, a health center medical director, and a personal background similar to the patients the NHSC strives to serve.

USC Dental Clinic at Union Rescue Mission Wins 2005 NHSC Award of Excellence

It is estimated that in Los Angeles County, there are over 90,000 homeless people.  Five years ago, the University of Southern California School of Dentistry (USCSD) developed a partnership with the Union Rescue Mission (URM) to create a comprehensive dental care clinic within URM for the homeless men, women and children living in the central city Skid Row area — something that was not present at that time.  As a stark comparison, there were five non-profit medical clinics serving the indigent in Skid Row.
