This working paper profiles post-grant experiences of 99 Rural Health Outreach Grant Program consortia that received initial endorsement by the Federal Office of Rural Health Policy. It discusses procedures to systematically track the sustainability of programs that were implemented by former Grantees in their communities. The paper presents the four categories that were used to define post-grant sustainability which include robust capacity, moderate capacity, minimum capacity, and no capacity. It includes tables, figures, and references.
