NIOSH employees perform free industrial hygiene surveys of plants upon request of employers or employees. The process is initiated by requesting a Health Hazard Evaluation (HHE). The request form and a description of the program are available through the CDC Fax Information Service by calling 1-888-232-3299. The HHE request form is document 750000; the program description is document 750010. Some HHEs may not require a site visit by NIOSH researchers. Sometimes a referral or providing information is enough to address the concerns of the HHE request. Go to top of page |
