Integrated Querying and Reporting Service (IQRS)

The IQRS is a web-based service for querying and reporting by registered entities and authorized agents designated on behalf of registered entities. The IQRS also provides a variety of administrative and user services to entities and agents. User accounts must be created by the Entity Data Bank Administrator.

The IQRS is available on a daily basis. The site is offline for system maintenance nightly between 2:00 a.m. and 5:00 a.m., EST. Other scheduled maintenance and system downtime events are announced on the home page.


Create and Manage User Accounts

Every entity must maintain an Entity Data Bank Administrator account. For security purposes, the Entity Data Bank Administrator account is limited to administrator functions only (cannot submit queries and reports). Entity Data Bank Administrators that also query and report must create a separate user account to do so. The Entity Data Bank Administrator account may only create, edit, delete, and update the passwords for other user accounts for the entity.   • Specify Response Routing for Agent-Submitted Transactions

    The Entity Data Bank Administrator may choose to have query and report output routed to the entity, the agent, or both. In addition, entities also have the ability to select how they receive query and report responses to their agent’s submissions. For example, if an agent submits a successful query using the QRXS service, the entity can elect to receive the query response via the IQRS, QRXS or ITP.

  • Terminate Entity/Agent Relationships

    Agents as well as entities may terminate entity/agent relationships. Entities will be notified of the deactivation via Data Bank Correspondence.

  • Manage Entity Registration

    Entity Data Bank Administrators may update their profile through the IQRS on the Update Entity Profile screen. Some updates may require that a form be printed, signed, and mailed to the Data Banks before processing. The IQRS will alert Entity Data Bank Administrators when this is required.   • Renew Entity Registration

    To maintain the integrity and confidentiality of the information in the Data Banks, it is essential that the Data Banks periodically request that all registered entities review the statutory authorities for the National Practitioner Data Bank and the Healthcare Integrity and Protection Data Bank, certify their authority to participate, and complete the registration information.

    Periodically reviewing the legal definition of an eligible entity and the legal requirement is important, which is one reason why the Data Banks ask entities and agents to renew every two years. Another reason is that periodic registration renewals ensure that the confidential information on file for each entity is accurate and current. Entities who fail to complete their renewal will not be able to use the Data Banks until the registration renewal is complete. If your entity does not renew their registration within the specified time, your DBID will be deactivated and you will not be able to conduct any Data Bank transactions until you have renewed your registration. Manage Credit Card and EFT Payment Information

Entity Data Bank Administrators may store EFT and credit card information in the IQRS and assign entity users to use specific credit cards for query payment. This feature makes querying easier for users because, once this information is stored, users do not have to enter their credit card information each time they submit a query. View Data Bank Correspondence

Important Data Bank messages are sent to entities through Data Bank Correspondence. Messages relating to upcoming system improvements, credit card expiration, registration renewal and agent designations and confirmations are all communicated through Data Bank correspondence.

IQRS users can elect to receive e-mail notifications of important Data Banks events.

E-mail Notification Description
Report Change Notifications
You will be notified when a report is changed by the reporting entity or the subject of the report, and the report is available via the IQRS.
PDS Report Disclosure Notifications
You will be notified when the Data Banks receive a new report on a practitioner you enroll in the PDS, and the report is available via the IQRS.
Notification of a Query, Report, or PDS Enrollment Response You will be notified when a query, report, or PDS enrollment response is complete and is available via the IQRS. You may opt to be notified of all submissions by your entity and agent(s) or only your submissions.
Data Bank E-newsletters You will be notified when a new Data Banks e-newsletter is available.
Administrative Events Users will be notified of administrative events such as:
  • When you must renew your entity's registration with the Data Banks,
  • When the Data Banks successfully process your entity's registration renewal or entity profile update,
  • When there is an activation, deactivation, or update of an entity/agent relationship involving your entity.

Submit Queries and Reports and Retrieve Responses

  • Querying the Data Banks

    Eligible entities may query the Data Banks via the IQRS, QRXS or ITP. The query is processed against the Data Banks based on the entity's eligibility. Queries may be submitted in single-name or multiple-name (batch) format. Responses to each query are available electronically in PDF format within two to four hours of receipt.   • Reporting to the Data Banks

    Eligible entities may report to the Data Banks via the IQRS, QRXS or ITP. The report data is stored in the appropriate Data Bank based on the type of report and the action taken. Reports may be transmitted as single reports or multiple reports. Within two to four hours of receipt, a Report Verification Document in PDF format is available to the reporter. Enroll and Manage Subjects in the Proactive Disclosure Service Prototype (PDS)

IQRS users may enroll a single subject, stored subjects, or subjects from the subject database to the Proactive Disclosure Service Prototype (PDS). PDS subjects are managed and cancelled within the IQRS as well. For additional information, see the Fact Sheets on the PDS.

Retrieve Report Change Notifications in PDF Format

When a report is modified (e.g., corrected by the entity or disputed by the subject), a paper copy of the changed report is sent to all entities that previously received the Initial Report (within a three-year time frame). Users may view the changed report electronically through the IQRS, and may also elect to receive notification of the changed report via e-mail.

Manage Subject Database Information

Entities may establish a subject database to complete querying and reporting obligations more efficiently. The subject database is a feature of the IQRS that offers an easy method for keeping track of identifying information about the subjects used routinely for querying and reporting. Entities may also import formatted subject data into the IQRS, eliminating the need to retype any data. View Historical Query and Report Submissions

Registered entities may obtain a summary of historical queries and reports that were submitted by their entity since June 2000. This enhancement aids entities in verifying that specific queries and/or reports were submitted. For additional information, see the Fact Sheet on Historical Query and Report Summaries PDF Document.

Reset Expired User Account Passwords

Users are able to reset their Data Bank password without having to contact the Customer Service Center, if they have an e-mail address on file with the Data Banks and know their expired password.
