Purpose
To support the development of rural health networks, whose purpose is to increase access to care by overcoming the fragmentation and vulnerability of providers in rural areas.  Grant funds are used to support activities that strenghten capabilities of these networks.

Funding Preference
(1) Those applicants where the service area is located in officially designated health professional shortage areas (HPSAs) or medically underserved communities (MUCs) or serve medically underserved populations (MUPs);

(2) Applicants whose projects focus on primary care, and wellness and prevention strategies.

Will consider geographic location when deciding which approved applications to fund if the majority of approved applications are from a single State.

Additional Information on Eligibility:
 Public and non-profit private entities, including faith-based and community organizations. Applicants in rural counties or eligible rural census tracts within an urban county. Applicant providing services exclusively to migrant and seasonal farm workers in rural areas and is supported under Section 330G of the Public Health Service Act. Federally recognized Native American Tribe or Tribal Organization that will deliver services on a reservation or federally recognized Tribal lands.
