  Supplemental Forms for NELRP

  Should I review and make copies of the supplemental forms before I mail them to the NELRP?
  Answer
  Yes.  It is important to review supplemental forms for completeness and accuracy before you submit them to the NELRP.   Forms must be clearly printed in their entirety on separate sheets of white 8 ½” x 11” paper.  Incomplete forms and inaccurate information on forms will result in the applicant being deemed ineligible to participate in the NELRP.  A copy of the application documents that you mail to the NELRP should be maintained for your records and for future reference if needed.
