This location contains the various forms and information used by Local Boards and Local Recipient Organizations. |
When entering your organization ID, a Local Board would enter the jurisdiction ID, 123456 for example. For an LRO, enter 123456789, where 123456 represents the jurisdiction ID and 789 is your LRO number. Do not include the phase number or any dashes. |
