We may ask you to complete a survey on the HRSA Web Reporting System (WRS) web site. This information is used to improve the WRS and to identify any problems users may have with the system. This information is strictly voluntary and you are not required to complete it.

The WRS respects your concerns about maintaining the privacy of any personal information you provide to us, and we have taken reasonable steps to ensure that unauthorized persons cannot access any information you provide. We do not sell, rent, share, or otherwise willfully disclose e-mail addresses or other personally identifiable information shared on this site to any third party unless otherwise mentioned

We do not use "persistent cookies" on this web site.

When you visit the HRSA Web Reporting System web site, the following information is stored:

This information is used to measure the number of visitors to the various sections of the site and to help us make the site more useful to visitors. Data submitted for the AOR/SDSPR reports are used to complete the intended purpose of the report(s) and the operations of the WRS.

Unless it is specifically stated otherwise, no additional information will be collected about you.

Retention of Data Collected

The contact information will be used to properly disseminate important AOR/SDSPR information to individuals responsible for the completion and submission of the reports. Contacts that are associated with the submission of the AOR/SDSPR reports and the approval of the AOR/SDSPR reports will have their contact information archived with the submitted reports. Contact information of users who have access to the WRS is retained as long as necessary to maintain the WRS and to complete the AOR and/or SDSPR reports.

Data collected through user support calls and/or support emails will be kept for the purpose of addressing the support request of the end-user. The data collected will be archived indefinitely and may be referenced as needed if future problems arise.

Accessing Your Information

Users may access their contact information through email or support service in order to correct discrepancies and update records with contact information. Users may also update their contact information from within the web site.

Cookies

The WRS uses non-persistent cookies to manage a user's session and to complete transactions.  No personal information is stored in our non-persistent cookies. Information is retained only for the brief period of time necessary to make use of it during a single online session. The cookie is destroyed once the session is over.

Correspondence

If you choose to provide us with additional information about yourself through an e-mail message, form, survey, etc., we will only maintain the information as long as needed to respond to your question or to fulfill the stated purpose of the communication.

However, support questions and suggested improvements to the WRS may be archived for future reference
