Medicare Advantage Plan PHRs

My Medicare...My Health Records

Several Medicare Advantage or Part D Drug plans offer Personal Health Records (PHRs) to their members with Medicare. The use of PHRs can help you to better understand and manage your own health and health care. PHRs can also help you find out information about your medications, conditions, and to keep track of your health history. To find out if your Medicare Advantage or Part D Drug plan offers you a PHR, please visit MyMedicare.gov
