MEMORANDUM FOR DESIGNATED AGENCY ETHICS OFFICIALS

SUBJECT: Availability of new standard confidential disclosure form (SF 450), and limited local reproduction


As we recently announced, the SF 450 for confidential financial disclosure reporting is now stocked and available through the Federal Supply Service, using National Stock Number 7540-01- 351-9173, at $2.50 per package of 50. For agencies who placed timely orders, you may have already received your supply.

However, the second printing may not be available immediately, so if you are submitting new orders or your original requisition was not filled, you may encounter some difficulty in obtaining an adequate supply promptly. This could occur because the second printing must be reset to correct a number of technical irregularities which we discovered in the initial delivery.

Because of this unusual circumstance, GSA has approved LIMITED local reproduction of the SF 450, pursuant to its authority at 41 CFR § 101-26.302. This authorization is valid only if an agency is unsuccessful in having its order filled through the Federal Supply Service by the end of October. Additionally, GSA has limited this local copying approval to the current cycle of confidential filings, through the end of calendar year 1992.

The local reproduction option will help alleviate an urgent need to have new entrants file now, before performing their responsibilities. Since agencies' old forms became obsolete on October 5, they have been unable to collect new entrant reports until the SF 450 is available.

Local reproduction of the SF 450 will also permit agencies to complete their annual filing procedure without further delay. Alternatively, your agency may still avail itself of the 30 day administrative delay which this Office announced by DAEO-gram of August 25, and it may also exercise authority under § 2634.903(d) of the new regulation to grant filers an extension of up to 90 days for good cause, such as form unavailability.

If you do choose to locally reproduce the SF 450 for the limited purposes specified above, you may use the advance copy which we provided to you by DAEO-gram of August 25, or contact your desk officer or the general counsel staff for another copy.
