Veterans who are interested in any of the programs below must
first be enrolled in the VA Hospital System. They then need
to speak with the social worker on whatever Primary Care Team
they are on (Red, White, Blue). The
social worker will then initiate the consult for them.
Adult Day Health Care Program
The Adult Day Health Care Program
is a program that provides nursing assistance, organized
activities, and caregiver relief to qualified disabled veterans
during daytime hours. The program runs generally from 7:30am
to 5:30 pm, Monday through Friday. There are two facilities
that are used to provide these services, one of which is
located in Shreveport, LA and the other in Longview, TX.
In Home Respite Program
The In Home Respite Program is a program that
is designed to assist caregivers of veterans that need help with
daily care and are not able to be left safely in their homes
alone. This program allows the caregiver time away from caregiving
duties.
Hospice
Hospice is a program that provides specialized
care to veterans who are near the end of their life. This care
may be provided in the veteran’s
home or in a hospice facility. Services include symptom management,
emotional and spiritual support for both the patient and
the family.
Homemaker / Home Health Aide Program
The Homemaker /
Home Health Aide Program is a program that is designed to
assist eligible veterans with certain chores at home. The
program may include help with light housekeeping (dusting,
changing linen, and vacuuming), meal preparation and personal
care. The housekeeping will be limited to the area that the
veteran utilizes such as his bedroom, bathroom, den, and
kitchen. Veterans referred to this program should be home
bound and have dependency with one or more of the following
areas: bathing, dressing, toileting, transferring and/or
feeding.
Home Based Primary Care Program (HBPC)
The Home Based Primary
Care Program (HBPC) brings primary healthcare services into
the homes of veterans who have chronic illnesses and have
difficulty coming to the hospital for appointments. Home
visits are made by nurses, dietitians, kinesiotherapists,
social workers and psychologists. Doctors and pharmacists
are also available to assist the team.
