• allopathic (MD) or osteopathic (DO) physicians
• primary care nurse practitioners
• certified nurse-midwives
• primary care physician assistants

Dental health care clinicians

• general practice dentists
• registered clinical dental hygienists

Mental health care clinicians

• psychiatrists
• health service psychologists
• clinical social workers
• psychiatric nurse specialists
• marriage and family therapists
• licensed professional counselors
