National Incident Based Reporting System Form

This is an example of the National Incident Based Reporting System (NIBRS) standard form developed by the FBI.  It provides the same information as the SHR form and much more specific information about assaults, sex offenses and homicides.  It describes whole incidents rather than single victims. This form can capture multiple circumstances and multiple victim-suspect relationships.
