FOIA Request Form

This form can only be used to request information from the Federal Law Enforcement Training Center (FLETC) under the FOIA.  We will not accept PA requests using this form. In addition, if the requested records contain personal information, we may ask you to establish your identity by requesting the information in writing and signing the request by hand.

This form collects information that is protected under the Privacy Act (i.e., home address and home phone).  Please read the Privacy Act Statement below.

I am requesting, under the Freedom of Information Act (FOIA), the following records:  (Be as descriptive as possible when describing records.  Include such items as subject, contract number, time frame, author, and recipient.  Also include the type of record i.e., memorandum, letter, report, form, etc.  Remember, the FOIA is not a forum for answering questions.   Please direct questions to the appropriate office listed here.)

Privacy Act Statement

This statement is provided in accordance with the Privacy Act of 1974, 5 U.S.C. 552(e)(3). Authority for the collection of this information is the Freedom of Information Act, 5 U.S.C. 552.  We will not use this information for any purpose other than the processing of your FOIA request.  Disclosure of this information is voluntary; however, failure to do so may result in our inability to process your request.
