SUBJECT:  Availability of the SF 450 (Executive Branch Confidential
          Financial Disclosure Report Form)

     By DAEO-gram of August 25, 1992, this Office provided you with
an advance black-and-white copy of the new confidential financial
disclosure form, the SF 450.  As we indicated, it will be stocked
through the Government supply system in late September or early
October.

     We have been advised by GSA that agency requisition requests
may be submitted at any time, and they will be filled as soon as
the forms are available.  Based on that information, you may wish
to place orders now through GSA's Federal Supply Service, using
FEDSTRIP or MILSTRIP format, as appropriate.  The National Stock
Number for this standard form is 7540-01-351-9173.  It will be
issued in packages of 50, at a price of $ 2.50 per package.  

     This new form has a green overlay and bears a date of 7/92.
