The financial disclosure report forms, both the OGE Form 450 and the SF 278, are provided below in various versions that are suited to individuals with different reporting requirements. Please read the instructions fully before choosing the appropriate form for your own needs.

How to add additional sheets to the Excel form

Annual Post-Employment Certification Subsection 8-400 of DoD 5500.7-R, Joint Ethics Regulation, now requires members of the Department of Defense who file a public financial disclosure report (SF 278) to certify each year that they are aware of the restrictions that three statutes place on them during Federal service when negotiating employment, and after departing Federal service. They must also certify that they have not knowingly violated these statutes. This Annual Post-Employment Certification accomplishes that task. It also meets the required notification to senior officials of post-Government employment restrictions under 18 U.S.C. Section 207.

OGE FORM 450 (CONFIDENTIAL FINANCIAL DISCLOURE REPORT)

(June 2008 Edition) (Electronically Fillable with Automatic Generation of Continuation Pages). Although this version of the form is capable of accepting digital signatures, the Office of Government Ethics has not authorized any agency to accept forms with digital signatures except those participating in an OGE-approved pilot program. Each filer must print a paper copy of their form, place their actual signature on the form and submit the signed paper form to teir designated supervisor or reviewing official for signature. As your supervisor / reviewer holds a legal obligation to fully review the financial disclosure reports of their employees, it is critical that you read through the entire form, gather your financial records that contain the information requested by the form, and then fill out the form. IMPORTANT: Do not send your completed OGE Form 450 to the SOCO Office. Your agency is responsible for reviewing each report for administrative sufficiency (proper position titles, phone numbers, completed entries and proper signatures) for each of their employees. Contact your agency ethics official to obtain the correct address for distribution or mailing of your form.

Note for Ethics Officials Concerning OGE-Approved Changes in the Administration of the Confidential Financial Disclosure Report Procedures: In 2006, the Office of Government Ethics changed the filing period for the OGE 450 to a calendar year reporting cycle. This change alters much of the procedural guidance in the Joint Ethics Regulation (JER). To clarify application of the new procedure, the DoD General Counsel issued a memorandum describing the effect of the superseding regulations on the JER. Our office produced a summary of the new procedures in a “page-by-page” format suitable for inserting in your JER along with expanded practical guidance on running the OGE 450 program during the transition. Attached here are the General Counsel’s memorandum, its enclosure listing the OGE regulations and superseded JER sections, the page-by-page listing of affected JER sections, and the program guidance.
