  4. Project Description on Experience to Date

    1. Project Description

      1. Problem:

        Create an infrastructure where data can be stored to introduce a seamless data link to analyze birth, death, Medicaid and newborn screening information. Conduct a survey of recent mothers to determine strengths and weaknesses in the health care delivery system and whether or not disparities exist.

      2. Goals and Objectives: List up to 5 major goals and time-framed objectives per goal for the project.

        Goal 1: Assess the health status of Delaware's children and families through the utilization of a variety of data sources.
        Objective 1: By September 29, 2006, develop a strategic plan to streamline data linkage process between birth records and Medicaid eligibility or paid claims
        Objective 2: By September 29, 2006, continue monitoring data linkages between birth recoreds and WIC eligibility files to identify process improvements and other potential partners
        Objective 3: By September 29, 2006, continue monitoring data linkages between birth records and newborn screening files.
         
        Goal 2: Promote effective use of data and information by state agencies, local health care advocates and community-based organizations for improving needed health and social services.
        Objective 1: By September 29, 2006 continue monitoring new mother data to identify trends.
        Objective 2: By September 29, 2006 continue survey of adolescent health and behaviors to assess data trends and implement public health programs to address identified needs.
         
      3. Activities/Methodology undertaken to meet project goals:

        The Delaware Division of Public Health's Community Health Care Access Section includes the Health Systems Development Branch and the Family Health Services Branch (FHSB). The HSDB manages the SSDI Grant, as well as the Primary Care Office and the State Office of Rural Health; the MCH Block Grant, CSHCN, and the SISS child care grant, are managed by the FHSB. The Branch directors meet regularly to collaborate in planning activities, monitoring progress and evaluating program outcomes. The SSDI program manager will solicit comsumer and provider input through routine participation in meetings, steering committees and other activities regarding health care systems needs and issues.

      4. The first three Healthy People 2010 objectives which this project addresses are listed below.


      5. Coordination: List the State, local health agencies or other organizations involved project and their roles.

        The progam manager will assure collaboration with and other state and private agencies, as well as other CHCA Branches which include the HSDB, FHSB, Women's and Reproductive Helath Branch, and Special Populations Branch. The HSDB manages the SSDI Grant as well as the Primary Care Office and the State Office of Rural Health. The MCH Block Grant, including the MCH Center for Excellence, Infant Mortality Task Force, CSHCN, and the CISS grant aremanaged by the FHSB.

      6. Evaluation : briefly describe the methods which will be used to determine whether process and outcome objectives are met.

        Evaluation of the SSDI [program will be based upon meeting the goals and objectives of the grant and fullfillment of the TItle V Block Grant performance outcome measures. The goals and objectives of the work plan have timeframes and measurable goals with tasks and performance measures for meeting each of the objectives. These tasks and performance measures are reviewed on a monthly basis.

    2. Continuing Grants ONLY

      1. Experience to Date :

        The SSDI program manager along with the Title V Block Grant manager established a PRAMS new mother survey pilot project. 300 surveys were sent to new mothers. The survey includes questions that directly target women's backgrounds, knowledge and resources. Questions focus on preconception, prenatal, postpartum and newborn care topics. How these are paraphrased will provide valuable information on gaps in understanding, personal issues, intendedness of pregnancy, support systems and barriers to care. The SSDI manager along with the Title V Block Grant Manager, Delaware Healthy Mother and Infant Cosurtium and Infant Mortality Task Force has submitted a Business Case Summary to the Delaware Department of Information & Technology to request support regarding the linking of health data to improve public health by more efficiently and expeditiously meeting health care needs, including preventive and therapeutic services, while maximizing often limited resources and provide linkage to newborn screening case management system in order to improve planning for infant mortality.

    The purpose of this grant is to improve access to primary health services for Delawareans through the establishment of data linkages. Both internal and external maternal and child health program staff needs access to data to evaluate and enhance their programs and services to Delawareans.
